Hello, I'm Dr. Gary Cash, and today we are going to discuss how to whiten really yellow teeth. While bleaching systems such as take home bleach, light based in-office systems and other in-office bleaching systems can be effective, sometimes the patient is really trying to achieve whiter results or sometimes we're coming from a starting point that is so yellow that those systems will just not achieve what the patient is looking for. In these cases, cosmetic dentistry is usually indicated. Your dentist should discuss risks and benefits of cosmetic dentistry before treatment starts. Options are veneers, crowns, and we have a wide range of materials we can use, all porcelain, porcelain metal, restorations, contact lens thin veneers and thicker veneers. Veneers that are adhered to the front of your tooth and they are permanently fixed and they are designed not to come off. Other things that may need to be done and certain indications are full coverage porcelain crowns. When we do these types of procedures, the dentist and patient have total control over how white we want to get. Results such as this on my left are achievable with home bleaching systems and achievable with in-office systems but patients wanting to whiten really yellow teeth and get from a position of this to this generally require these porcelain restorations. Other more conservative options are tooth colored filling materials that can be placed on the front of the tooth in certain indications. However, patient should note these do not last as long as porcelain restorations and tend to pick up staining over time and need to be done at a more frequent interval. They tend to last on the order of five to ten years whereas the porcelain restorations we'd like those to last ten sometimes 20 years. All of these cosmetic dental options may not last a lifetime and may need to be redone over time. These are risks and benefits that you really should discuss with your dentist before consenting to treatment.
